CHILD CUSTODY FORM

A child custody form is something that must be filed to determine which parent has custody of a child. This form should be filled out with a divorce and if you have never been married if the father’s name is on the birth certificate.

When you go through a divorce, you will have to fill out a child custody form. This is information about both of the parents and the child’s place of residency since birth, information that will help the courts determine stability, and more.

If you were never married but you had a baby and the father’s name is on the birth certificate, then you must fill out a child custody form. Let’s say that the child has lived with you all of his or her life until six years of age and then all of the sudden, the father has decided that he wants to see the child and he wants the child to live with him. The father could take the child from you and not bring the child back to you until something was filed through the courts. The fathers name on the birth certificate gives him the same rights to the child as you. Even if he has never seen the child before. This could leave you in a situation that you don’t know how to find the father, where he lives, or any information. He just showed up one day at school and grabbed the child. This is not kidnapping or against the law. There is nothing that law enforcement will do for you nor is there anything the courts will do for you. In addition, you cannot file a child custody form unless you can have the father served with papers. That means you have to know some type of information on the father with his whereabouts.
